It can be tricky to check that you've got all the forms you need as a learning provider, because they relate to different topics and stages. To make it easier, we've gathered all the Skills Development Scotland forms that you or the apprentice will need to complete during their apprenticeship.
Training Agreement
Appendix 5 – Training Agreement, 2024-25 (PDF) — complete this document to inform Skills Development Scotland about a learner's training and employment arrangements.

Summary of Achievement Record (SOAR) forms
The Summary of Achievement Record (SOAR) is the document you’ll use to record an apprentice’s completed units and submit achievement claims to SDS. It ensures that learning is clearly evidenced and signed off by a qualified assessor.
